I signed up for Desire Luzinda's daughter's fitness program, here's what I found

Michelle Kaddu is Desire Luzinda's 18-year-old daughter. Kaddu has been raised by Luzinda in California, United States, and graduated from high school last year. She is looking at joining medical school at university not unlike her mother who is a trained phlebotomist. She also shares her mother's passion for fitness and this editor signed up for her fitness program.

Kaddu lives by two words – faith and fitness, according to her social media bio at least. Her favourite bible verse Deuteronomy 3:16 also seems to back up her world view, "Be strong and of good courage, fear not, nor be afraid of them: for the LORD thy God, he it is that doth go with thee; he will not fail thee, nor forsake thee."

It is little wonder then that the teenager took up fitness mentorship to help other fitness lovers.

Kaddu used the Jot Form instead of the typical Google form. The Jot form comes with many perks that the Google Form doesn't have especially for commercial services. Notably, it offers payment integrations which are not found in Google Forms.

According to the Jotform Company, using the Jotform can cost anywhere from Shs140,000 monthly to Shs4 million annually. However, it also has a free pack albeit with limited provisions.

Her personalised form has 24 fields that take information about your personal details and fitness needs and inquiries.

You are required to submit your name, email, gender, and phone contact. There's no field for date of birth.

The following fields ask about fitness goals including whether you want to lose or gain weight or simply live healthy.

Perhaps the weirdest question is whether or not you have used Herbalife and how long ago you used it. Herbalife (Herbalife Nutrition Ltd., Herbalife International, Inc.) develops and sells dietary supplements since 1980 when Mark R. Hughes founded it.

According to a Healthline dietician, the Herbalife diet is designed to help people lose weight by reducing calorie intake with meal replacement shakes and boosting metabolism with supplements.

However, it was given a 1.79 out of five diet score with the review saying, "The Herbalife diet is pricey and involves highly processed shakes and many supplements, some of which have been linked to negative health effects. Short-term use likely causes weight loss, but long-term effectiveness is yet to be studied."

The next field on the form is the financial investment one is willing to inject. The options are as follows; $50 - $100, $150 - $200, and "Money isn't an issue."

Next up, you are required to submit your country of residence and Instagram user name.

This is followed by the question, "If you want to lose body fat, how much do you want to lose and what have been some hardships you faced when trying to achieve your body goal?" and "Any questions for your future coach or anything you feel like they need to know about you?"

After clicking on "Submit" you are taken to a CAPTCHA which you have to do three times before finally submitting the form.

Different fitness programs have different forms and require different information like this one. For some, simple information is required before deeper inquiries are made and a program is structured.

Kaddu is attending the University of Nevada, Reno gathering from the "UNR🐺" in her Instagram bio. The university's mascot is a wolf (by the names Wolfie Jr., Alphie and Luna).
